If you want to get a US passport, you must prove that you have US citizenship. The first time you apply for a passport, you’ll have to fill out form DS-11, then gather and copy official documents to establish who you are and the fact that you are a U.S. citizen. You can fill out the application online or by hand.
To fill out the application online, use the Passport Application Wizard to step through the process of filling out the application. When you’ve completed the application, print it out to take to the passport acceptance facility. Passport applications cannot be submitted online.
To fill out the application by hand. Download the passport application form (DS-11) and instructions (PDF, Download Adobe Reader) and complete the application by hand, or go to a local passport acceptance facility to get a copy to fill out.

To prepare personal documents, you must collect three types documents including:
➣ An original proof of citizenship document
➣ An acceptable photo ID document
➣ A photocopy of the front and back of the citizenship document and photo ID document
A single 2-by-2-inch passport photo must be submitted with each application. Passport photos must be in color and taken within the past six months in front of a white or off-white background. Applicants should look directly into the camera with a neutral expression and with the head taking up the majority of the frame. Nothing should cover your hair unless you provide a signed statement that the head covering is for religious purposes. No glasses also. The correct size is 2 x 2 inches (51 x 51 mm).
Calculate your fees. You’ll also need to pay an application fee to the U.S. Department of State. In most cases, the application fee must be made with a check or money order.
